Threshold, Kelvin etc: Those cities have their own penal code. This is purely for the Mirros and surrounds.
It's also only relevant for criminals and law enforcement characters. This info would be available in libraries and archival systems throughout the country, relating mostly to crimes in Mirros and the justice system there.
Some cities may decide to take on the well tested and regarded system of Mirros as their own, however.

I'd argue that the Karameikos Penal Code is in effect across the entire country as its effectively a set of national laws enforced by the King's Armed Forces. They're just the base of whatever local laws are in place (e.g. Mirros = Karameikos Penal Code; Threshold = Karameikos Penal Code + Threshold Laws), same as federal and state laws are IRL.
However, given the current state of affairs in the country, the KAF don't have the capacity to enforce any of those laws beyond their current area of influence... unless its something serious, like say, treason and they're currently combating a private militia to arrest those responsible. A hypothetical situation, of course.
